The SANE Certification Commission must receive these documents to consider your application.

Step 3: Start Application

  • There will be a link within the application for payment.

  • All required documents must be submitted as a single PDF file.

  • Resources and instructions for combining files can be found HERE.

  • (1) Copy of your SAE/SANE training certificate. If completed out of state or through IAFN, also include OR SANE Connect certificate.

    2) Initial Certification Packet: Required Clinical & Non-Clinical Validation Forms, Reflection prompts.

    (3) Three redacted Sexual Assault Medical-Forensic Exam forms. Each form must include a corresponding Treatment/Follow-up document.

    SA Exam Forms

  • (1) Recertification Packet: CE Log, Active Practice Forms, and Reflection Prompts.

    (2) One redacted Sexual Assault Medical-Forensic Exam form, including the corresponding Treatment/Follow-up document.

  • (1) Copy of your out of state/IAFN SAE/SANE certification, as well as your OR SANE Connect certificate.

    2) Out-of-State Transfer Certification Packet: Required Clinical & Non-Clinical Validation Forms, Reflection prompts.

    (3) Three redacted Sexual Assault Medical-Forensic Exam forms. Each form must include a corresponding Treatment/Follow-up document.
